The number of children with chronic critical illness (CCI) is a growing population in the United States. A defining characteristic of this population is a prolonged hospital stay. Our study assessed the proportion of pediatric patients with chronic critical illness in U.S. hospitals at a specific point in time, and identified a subset of children whose hospital stay lasted for months to years. The potential harms of a prolonged hospitalization for children with CCI, which include over treatment, infection, disruption of (...) family life, and the intensive utilization of resources—combined with the moral distress experienced by the clinicians who care for the children, suggest the need for ethical analysis of this growing issue to identify actions that could be taken at the clinical and health systems levels to reduce the harms associated with prolonged hospital stay.In this article we present three real cases from our study that involved a very long hospital stay. We applied a framework developed by Mackenzie, Rogers, and Dodds to analyze inherent, situational, and pathogenic vulnerabilities to examine the ways that interventions intended to remedy one source of harm for the children in our cohort inadvertently created other harms. We examined the complex ways that children with protracted hospitalization are vulnerable to the choices made by their family and clinicians, as well as by healthcare systems and communities. Finally, we used this analysis to summarize actions and ethical responses to this growing patient population. Such an understanding is essential to making clinical and ethical decisions that arise for children who are at risk for a very long stay in the hospital. (shrink)

Caring for critically ill and dying patients often triggers both professional and personal growth for physician trainees. In pediatrics, the neonatal intensive care unit is among the most distressing settings for trainees. We used longitudinal narrative writing to gain insight into how physician trainees are challenged by and make sense of repetitive, ongoing conflicts experienced as part of caring for very sick and dying babies. The study took place in a 45-bed, university-based NICU in an urban setting in the United (...) States. From November 2009 to June 2010 we enrolled pediatric residents and neonatology fellows at the beginning of their NICU rotations. Participants were asked to engage in individual, longitudinal narrative writing about their “experience in the NICU.” Thematic narrative analysis was performed. Thirty-seven physician trainees participated in the study. The mean number of narratives per trainee was 12; a total of 441 narratives were available for analysis. Conflict was the most pervasive theme in the narratives. Trainees experienced conflicts with families and conflicts with other clinicians. Trainees also described multiple conflicts of identity as members of the neonatology team, as members of the medical profession, as members of their own families, and as members of society. Physician trainees experience significant conflict and distress while learning to care for critically ill and dying infants. These conflicts often led them to question their own morals and their role in the medical profession. Physician trainees should be educated to expect various types of distress during intensive care rotations, encouraged to identify their own sources of distress, and supported in mitigating their effects. People’s comfort with and acceptability of artificial intelligence (AI) instantiations is a topic that has received little systematic study. This is surprising given the topic’s relevance to the design, deployment and even regulation of AI systems. To help fill in our knowledge base, we conducted mixed-methods analysis based on a survey of a representative sample of the US population (_N_ = 2254). Results show that there are two distinct social dimensions to comfort with AI: as a peer and as a (...) superior. For both dimensions, general and technological efficacy traits—locus of control, communication apprehension, robot phobia, and perceived technology competence—are strongly associated with acceptance of AI in various roles. Female and older respondents also were less comfortable with the idea of AI agents in various roles. A qualitative analysis of comments collected from respondents complemented our statistical approach. We conclude by exploring the implications of our research for AI acceptability in society. In fact, Optimism is not the invention of Leibniz', but of the two Sevillan Jesuits' Diego Ruiz de Montoya and Diego Granado . It will be argued that Optimism has two roots, both of them closely connected with the Posttridentinian Theology: 1) the moralization of modal categories, 2) a universe integrating (...) Nature, Grace and Incarnation. In the latter regard, Optimism is a legitimate child of Molinism, viz. in respect to the ,scientia media‘ being the basis of its concept of World, to the ,total decree‘ as a certain mode of this world being realized. As a result, the Théodicée depends on Jesuit theology to an extent which still has to be acknowledged. (shrink)

The anti-realist argument from underconsideration focuses on the fact that, when scientists evaluate theories, they only ever consider a subset of the theories that can account for the available data. As a result, when scientists judge one theory to be superior to competitor theories, they are not warranted in drawing the conclusion that the superior theory is likely true with respect to what it says about unobservable entities and processes. I defend the argument from underconsideration from the objections of Peter (...) Lipton. I argue that the inconsistency that Lipton claims to find in the argument vanishes once we understand what the anti-realist means when she claims that scientists are reliable. I also argue that collapsing the distinction between relative and absolute evaluations, as Lipton recommends, has its costs. Finally, I briefly examine Richard Boyd's influential defence of realism. When we understand the actions of another person, do we do so on the basis of a general theory of psychology, or on the basis of an effort to place ourselves in the particular position of that specific person? Hans Herbert Kögler and Karsten R. Stueber's Empathy and Agency addresses this other issues vital to current (...) social science in an advanced and diverse analysis of the foundations of social-scientific methodology based on recent cognitive psychology. This essay discusses an alternative interpretation of the term “Dasein” as Heidegger uses it in Being and Time and, in particular, the possibility that Dasein is meant to contain an inherent form of intersubjectivity to which we must “return” in order to achieve authenticity. In doing so, I build on the work of John Haugeland and his interpretation of Dasein as a mass term, while exploring the implications such an interpretation has on Heidegger’s conception of “authenticity”. Ultimately, this paper aims (...) to take seriously Heidegger’s claim to be moving past the isolated Cartesian subject and towards a view of authentic human existence that is cognizant of the way our identities are always formed within a pre-existing community. In addition, since many interpretations of Heidegger have argued that “the Anyone” is representative of all possible forms of community, I consider how this alternative understanding of Dasein as intersubjective can shed new light on critical remarks Heidegger makes about “the Anyone”. Thus, I argue that by reinterpreting Dasein as community, we can find more coherence between Heidegger’s otherwise conflicting conceptions of authenticity and “the Anyone”. (shrink)

We examine the sub-field of philosophy of science using a new method developed in information science, Referenced Publication Years Spectroscopy (RPYS). RPYS allows us to identify peak years in citations in a field, which promises to help scholars identify the key contributions to a field, and revolutionary discoveries in a field. We discovered that philosophy of science, a sub-field in the humanities, differs significantly from other fields examined with this method. Books play a more important role in philosophy of science (...) than in the sciences. Further, Einstein’s famous 1905 papers created a citation peak in the philosophy of science literature. But rather than being a contribution to the philosophy of science, their importance lies in the fact that they are revolutionary contributions to physics with important implications for philosophy of science. DaB fUr die Entwicklung der Phanomenologie Husserls nicht zuletzt auch,wissenschaftstheoretische' Motive ausschlaggebend waren, ist hinreichend bekannt. Umso mehr verwundert es, daB die Husserl-Rezeption sich diese Motive nur selten zueigen gemacht hat. Zwar sind in einer Reihe von Einzel­ wissenschaften, wie in der Literaturwissenschaft, der Sozialwissenschaft und besonders auch der Psychologie, phanomenologische Motive wirksam gewor­ den. Versuche einer ausdriicklichen wissenschaftskritischen Anwendung der Husserlschen Philosophie auf einzelwissenschaftliehe Theorien jedoch, wie sie z.B. Alfred Schiitz in seinem friihen Werk zur phanomenologischen Grundlegung der "verstehenden (...) Soziologie"l ansatzweise unternommen hat, sind rar. Freilich ging auch Husserl hier nieht eben mit gutem Beispiel voran. Auch er selbst hat seine wissenschaftstheoretische Programmatik nirgends einer ernsthaften Bewahrungsprobe unterzogen. Die vorliegende Untersuchung will diese Frage einer wissenschaftstheore­ tischen Anwendbarkeit der transzendentalen Phiinomenologie paradigmatisch im Hinblick auf die Problemstellungen der Geschiehtswissenschaft iiberprii­ fen. This work is an exposition of the philosophic conceptions basic to Mahayana Buddhism as found in the Maha-prajnaparamita-sastra a commentary on the Prajnaparamita-sutras and traditionally attributed to Nagarjuna. The sastra the earliest and most extensive work in this field is lost in its Sanskrit original and preserved only in a Chinese translation. My aim in this paper is to re-examine specialization in science. I argue that we need to acknowledge the role that conceptual changes can play in the creation of new specialties. Whereas earlier sociological accounts focus on social and instrumental changes as the cause of the creation of new specialties, I argue that conceptual changes play an important role in the creation of some scientific specialties. Introduction of deep neural networks to the machine translation research ameliorated conventional machine translation systems in multiple ways, specifically in terms of translation quality. The ability of deep neural networks to learn a sensible representation of words is one of the major reasons for this improvement. Despite machine translation using deep neural architecture is showing state-of-the-art results in translating European languages, we cannot directly apply these algorithms in Indian languages mainly because of two reasons: unavailability of the good corpus and (...) Indian languages are morphologically rich. In this paper, we propose a neural machine translation (NMT) system for four language pairs: English–Malayalam, English–Hindi, English–Tamil, and English–Punjabi. We also collected sentences from different sources and cleaned them to make four parallel corpora for each of the language pairs, and then used them to model the translation system. The encoder network in the NMT architecture was designed with long short-term memory (LSTM) networks and bi-directional recurrent neural networks (Bi-RNN). Evaluation of the obtained models was performed both automatically and manually. For automatic evaluation, the bilingual evaluation understudy (BLEU) score was used, and for manual evaluation, three metrics such as adequacy, fluency, and overall ranking were used. Analysis of the results showed the presence of lengthy sentences in English–Malayalam, and the English–Hindi corpus affected the translation. Attention mechanism was employed with a view to addressing the problem of translating lengthy sentences (sentences contain more than 50 words), and the system was able to perceive long-term contexts in the sentences. Perhaps where does not express the epistemic possibility of ‘Michael led the league in scoring’, it expresses the epistemic possibility of ‘Michael would have led the league in scoring’. Analysis of this ‘would have’ statement, together with an exploration of the suspicion that the ‘might have’ statement is ambiguous between these two epistemic possibilities, will have to await another occasion. The important point for present purposes is that is ambiguous between an expression of the epistemic possibility of ‘Michael led the league in scoring’ and some quite different reading. (shrink)

Through qualitative surveys, a team of law students, law professors, physicians, and residents explored the perceptions of neurology residents towards referral to appropriate legal resources in an academic training program. Respondents reported feeling uncomfortable screening their patients for health-harming legal needs, which many attributed to a lack of training in this area. These findings indicate that neurology residents would benefit from training on screening for social factors that may be impacting their patients’ health.

Irreversibility, it is claimed, is a much broader concept than is entropy increase, as is shown by the occurrence of certain processes which are irreversible without seeming to involve any intrinsic entropy change. These processes include the spreading outwards into space of particles, or of radiation, and they also include certain biological and mental phenomena. For instance, the irreversible and treelike branching which is characteristic of natural evolution is not entropic when it is considered in itself—i.e. in abstraction from accompanying (...) biochemical and physiological activity. What appears to be the common feature of all forms of irreversibility is the fanning out of trajectories, new entities or new states, in the temporal direction towards the future. (shrink)
